BotRK, IE, Zephyr/PD (what you prefer) in that order, with Greaves/LW/GA in between depending on the importance of it.

I've had 2 (MF & Twitch), but one was on a smurf so I'm not really sure if it counts.

I also had one stolen from me when I already had a quadra, the fifth was in AA range at less than 100 HP, and Thresh just AA'd it. What a jerk.
