They release small changes and new champions throughout the year, but every year they tend to make larger changes at the season reset, which is also when your ranking resets and when the pro stuff starts over. S2 looked very different from the current S4.

no you stay at your current level, masteries, champions, and rune pages

it's just your Ranked ELO that gets reset
