Captcha for too many login attempts is broken

When you try to login in with an incorrect password (Say if someone forgot to authorize your alt and you no longer remember the password a few weeks later), It asks you for a captcha.

The captcha does not work and shows either a broken image link (PC) or simply the words "Confirmation code." (Mobile)

Possibly as a result, the reset password email never arrives.

Got the password reset, but now the captcha shows up and isn't accepted, even when entering it exactly.
